This letter is to state our objection to any form of immunization for our child, (child?s name), based on religious grounds.

We believe that as our Creator, God put within our bodies healing capabilities. We are required by Him to keep our bodies in optimum condition in order to allow this healing mechanism to function properly. We believe that this entails avoiding any substances or drugs viewed by our bodies as harmful, regardless of man's traditional views on these same substances. After much research and investigation into the side effects and sources of them, we further believe that vaccinations / immunizations are included in the list of toxic substances / drugs that we are to avoid.

Based on the policy of religious exemption to immunization as applied to school children sent to us from the Indiana State Department of Health, we need only write this letter, sign it and give it to you or his teacher each year.
